Accepted Insurance Plans by State

First Care accepts most major health insurance plans and can bill the insurance companies directly. To use your insurance, bring your primary insurance card, secondary insurance card (if you have one), and the policyholder’s name, date of birth, and social security number. Most co-pay rates are equal to office visit or PCP amounts.

First Care is happy to be able to accept most Fee-for-Service Medicaid and Managed Medicaid insurance plans at all First Care Clinic locations. Please be advised that we do not accept out-of-state Medicaid. We are continually being added as a participating provider to health plans, so be sure to check our website regularly for updates!

Medicare

We accept Medicare and Medicare Railroad and most Medicare Replacement plans.

All First Care Clinic locations are happy to accept Medicare and Medicare Railroad, as well as most Medicare Replacement and/or supplemental plans. Please be advised that we do not accept out-of-state Medicare.

$65 Prompt Pay Program

During the COVID-19 pandemic, First Care is offering a temporary self-pay price of $65 for our uninsured patients. The $65 Prompt Pay Program allows patients to only pay $65 upfront, which includes an office visit fee, some in-house testing, and procedures. Please see our front desk team members for more information.

Patients who utilize their health insurance coverage for healthcare services will have all rates for such health services determined by the insurers and/or as otherwise determined by First Care for services provided outside of its prompt pay program.
