$89 Prompt Pay Program
Pay $89 Upfront for all In-House Tests and Procedures
For patients who either do not have health insurance or have a high-deductible health plan (HDHP) and prefer not to use their insurance, First Care offers competitive self-pay rates. The Prompt Pay Program allows patients to pay an $89 upfront office visit fee that includes all in-house testing and procedures.

|Blood Sugar / Glucose|
|Cut and Wound Closure (stitches)|
|Evaluation for Corneal Abrasion|
|Incision and Drainage of Abscesses|
|Injections (excluding immunizations)|
|Rapid tests for Strep, Flu and RSV|
|Removal of Foreign Body / Toenail Removal|
|Staple and Suture Removal|
|Therapeutic Blood Levels PT / INR|
|Treatment of burns|
|Treatment of fractures|
|Urine Pregnancy Test|
|Examples of services not covered in the $89 Prompt Pay Program office visit:|
|Drug Screening Services|
|Durable Medical Equipment (crutches, splints, etc.)|
|Immunizations (Influenza, Hepatitis B, TDAP, etc.)|
|Outside Lab Tests (CBC, Lipid Panel, Urine Culture, etc.)|
|School Physicals / Sports Physicals / DOT Physicals / Work Physicals|
|Services covered by worker’s compensation or other insurance|
Patients who utilize their health insurance coverage and/or worker’s compensation coverage for healthcare services will have all rates for such health services determined by the insurers and/or as otherwise determined by First Care for services provided outside of its prompt pay program.
Co-pay Rates Equal to Office Visit or PCP Amounts
First Care accepts most major health insurance plans and can bill the insurance companies directly. To use your insurance, bring your primary insurance card, secondary insurance card (if you have one) and the policyholder’s name, date of birth and social security number.
