What To Bring To Your Urgent Care Visit

We want your visit to First Care Clinics to be as quick and convenient as possible. In order to expedite your treatment, be sure to bring your driver’s license or picture ID, your insurance card (if applicable), payment for your copay, coinsurance, any unmet deductible amounts – which are due at the time of treatment, and a Parental Authorization Form if you are under 18. To provide the best care possible, we also suggest you bring a list of any current medications, information on any past surgeries or medical conditions, as well as a list of any allergies you may have. Having this information helps us continue to provide the most efficient and quality service to get you feeling better in no time.
